Schedule An Appointment With Us

Wall Family Chiropractic Center wants to be your chiropractic care provider in the Tacoma, WA area.

If you would like to schedule an appointment, please fill out all sections of the form. All appointments will be scheduled during business hours only, and if the time you are wanting for your appointment isn’t available, we will respond with an alternative. We will do our best to work with you on an appointment that fits your needs. As a way to expedite your wait time, please feel free to print off the forms below, fill them out, and bring them in with you on your visit.

Due to limited available new patient exam times, clients will be charged a “good faith” deposit of fifty dollars that will be applied to their first visit charges

No Show & Cancellation Policy

No Show & Cancellation Policy: If you are unable to keep your scheduled new patient appointment, it is the policy of Wall Family Chiropractic Center that you must cancel and/or reschedule your appointment at least 24 hours PRIOR to your scheduled appointment time.

Wall Family Chiropractic Center policy states that any client who fails to be present for their scheduled new patient examination appointment without the required notice, will be charged the deposit for their scheduled appointment. Without the required notice a fifty dollar fee will be charged for the missed appointment.

The term “No-Show” refers to an appointment missed by a client.

The term “Cancellation” describes an appointment canceled by a client with less than 24-hour notice.
